      The Clinic provides limited dental services to individuals accepted as patients. The services are best described as "getting patients out of pain" and are limited generally to extractions and fillings, though each dentist determines which services he/she is willing to perform. The Clinic does not provide cosmetic services, which means no dentures, partial dentures, whitening, braces or other specialty care.Clinic hours Monday – Friday  8:30 am – 4:00 pm (Closed for lunch) Phone   815-726-3377 Fax  815-726-2708 How do you become eligible for Services?Make an appointment to be screened by the Will-Grundy Medical Clinic to see if you qualify to become a patient.Bring the required information and answer questions related to your health during the screening.   The screening interview usually takes between 30 – 45 minutes. Required Information:Proof of income for all members of the household 18 years of age or older. If you get paid weekly you need to provide 4 currents paycheck stubs. If paid bi-weekly you need 2 current paycheck stubs.  If you are paid in cash a letter on company stationary with the daily hours and days per week worked is necessary. W-2 & 1040’s – all income tax information, the complete folder. If your children have a Medical Card you need to provide the current card. If you are on unemployment you need to provide the current statement. If your unemployment is exhausted you need to provide the last statement. If anyone in the household is on Public Aid, Social Security, Disability or SSI,             Food Stamps, a pension, township assistance, or child support, the current                information for these resources (monthly benefit) must be presented. The name of all medications that have been prescribed by a physician  The name & address of the physician(s) who have treated you and/or the hospital(s) where you have received medical attention. If you are a client of Daybreak, Morningstar Mission, Joliet Township, or Groundworks please bring your referral letter to the screening appointment. Proof of Residency:Proof that you are a resident of Will or Grundy counties. A picture ID with your name and current address.  If the ID does not have the             current address, you need to bring a utility bill, rent receipt, lease or other             document that has the name and current address of the person applying. Applications with missing financial and ID information will only be held for 30 days from date of application.Who is not Eligible?The Clinic does not serve individuals with the following situations: Acute or emergency care, birth control, pregnancy, dialysis,            infectious diseases, immunizations, alcohol or drug related diseases, multi-system             or complex medical needs, psychiatric care, sexually transmitted diseases or other              health care beyond the scope of the Clinic. If you have insurance, Medicare or Medicaid, U.S. Veteran Health Benefits, or a household income is over WGMC guidelines. If you have a work injuries or involved in a lawsuit for auto accident injuries.
