      The clinic offers the following On-site Services for free:Primary Care for:    Care for Acute illness such as pain, injury, virus and infection    Chronic Illness such as Asthma, Diabetes, Hypertension    Dental (for TCHP) clinic patients to include: Diagnostic, Preventive and Restorative Services    Flu Immunization    Healthy Lifestyles Programming    Basic Wellness checks & Health Screenings    Lab Draws    Medication Assistance    TB Screening    Women’s HealthAdditional services:    Acupuncture    Chiropractic    Dermatology    Healthy Lifestyles Programming    Mammography    Physical Therapy    Podiatry    Smoking Cessation    Vision    X-ray, Diagnostic Procedures and Limited SurgeryTo qualify for care:    Resident of central Kane County or work in central Kane County for a minimum of 20 hours per week.    Unable to afford medical services    Have an annual income at or below 250% of the Federal Poverty Guidelines:    For example, it is $60,625 for a family of four, and $29,425 for an individual.    Not currently enrolled in an effective third party insurance.
