Dental Services:  General Dentistry  Preventative care  Restorative care  Dental Hygiene  Dental care education CHC offers a sliding fee schedule to discount the cost of medical or dental care for those patients with incomes below 200 percent of the federal poverty line. All eligible patients who apply for this discount program must provide proof of income. Patients are expected to pay appropriate co-pays at time of service.
