Hours There are no set office hours. Call and leave a number with the answering service and your call will be returned Intake Process - Medical and Dental services are by appointment only - Must submit a completed qualification form before medical or dental treatment is offered - Photo ID - Proof of income with application - Verification if receiving SNAP benefits, Unemployment, one month pay-stub, child support, alimony, SS (retirement or disability) Veteran disability or TANF Program Fees No cost
