      This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Services: Preventative care Patient Eligibility: No patient criteria Fees: Those on Medical Assistance receive free care. The clinic also offers a sliding fee schedule and is an approved Medical Assistance provider. Work Hour   Monday through Friday, 8:30 a.m. - 11:30 a.m. and 1:30 p.m. - 4:30 pm.; Tuesday and Thursday evenings, 6:00 p.m. - 8:30 p.m.
