This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Sees patients in Lebanon, Marshfield, Springfield, Republic, Forsyth and Hollister for the convenience of their patients. Children and adults go to Jordan Valley for exams, cleanings, extractions, root canal therapy, crowns, partials, urgent dental needs, such as tooth pain and more. Jordan Valley offers a mobile dental unit program for school-aged kids that serves much of Southwest Missouri but also provides comprehensive dental care to special needs and nursing home patients. Sliding scale available to those who qualify. Proof of income includes all persons living in the household. Any household members determined to potentially qualify for Medicaid coverage are strongly encouraged to apply for Medicaid. The staff is available to assist you with this process.
