This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. A federally qualified health center. The clinic provides basic primary care, such as check-ups, school and work physicals, immunizations, blood tests, gynecological exams and treatment of minor ailments for children, adults and senior citizens. Clinic also provides well baby services. Patients who need specialist care are referred to doctors in the community. Care is provided by doctors and nurses, and medical assistants and bilingual translators. The clinic has a full time dental program, both of which requires a small patient co-payment. The center also provides case management, Medicaid enrollment service, English as a Second Language classes. However, walk-in emergency patients are seen for an evaluation, radiographs, prescription and/or palliative treatment. They are then scheduled for follow-up care.Procedure: New dental patients who are not currently registered and receiving medical care at Zufall Health must register prior to making their first dental appointment. Registration can be done on site at one of our locations in Dover, Morristown, or Flemington.Registration: New patients will need to bring:1. Identification2. Proof of income such as a pay stub, income tax return, or a letter of employment3. Verification of addressPatients without health insurance will be charged a sliding scale fee based on their ability to pay.First Visit: The first dental visit consists of a clinical exam, x-rays, and treatment plan. Patients are then scheduled for an appointment for a cleaning or periodontal scaling based on the condition of their gums. All patients should bring a current list of medications, allergies, hospitalizations, and medical problems for the first visit and inform the dentist of any changes at future visits.
